Emergency PPE Order Form
This form only for ordering emergency PPE for people entitled to PPE under local resilience forum (LRF) arrangements. For regular PPE supplies, please use the portal. Please note: there is no charge for the PPE
All orders need to be placed by 12pm on the day before collection and by 12pm on Friday for Monday collections. The hub coordinator will email you to confirm your order will be available and to clarify any items where needed. Please remember to bring a copy of your order and something to carry the PPE in when you collect. If you have any questions, please email covid19centralstockmanagement@ghc.nhs.uk
Hub opening days and times are as follows:
Monday
- Edward Jenner Court, Gloucester, 10am – 12pm.
Tuesday
- Cirencester Hospital, 11.30am – 12.00pm
Thursday
- North Cotswold Hospital, 10.30am – 11.00am
- Lydney Hospital, 11.45am – 12.15pm
The hub coordinator will email you to confirm your order will be available an to clarify any items where needed. They will also send you details of where to collect your PPE.
