If you have already returned a paper consent form, and your child was absent, unwell or refused on the day of the school-based session, please do not complete an e-consent and go direct to book an appointment.
If you did not respond to the initial offer and have not returned a paper consent form to school, you will be required to complete an e-consent as part of the booking process.
