Children’s Community Nursing

Our Children’s Community Nursing Team (CCNT) provides a community-based nursing service for children and young people, up to the age of 18, with specific medical conditions requiring nursing treatment.

About this service

It is a countywide specialist nursing service for children and young people who have a designated consultant paediatrician managing their ongoing care. We provide support in the most appropriate place for each individual. This may include schools, clinics, nurseries or in the child’s home.

We help to support early discharge from hospital or prevent hospital admission where possible. Our nursing care will enable our patient’s individual acute and complex health needs to be managed in their own home or in a clinic setting, where appropriate. We also provide transitional care for those moving into adult services.

We specialise in:

  • Gastrostomy management
  • Nasogastric care
  • Wound care
  • Assessment for specialised bed and gastrostomy equipment
  • Intravenous medication
  • Subcutaneous injections for conditions
  • Cardiac care following discharge from regional centre
  • Venepuncture
  • End of life care to children in their own homes.

Referrals

We accept referrals from GPs, consultants or other healthcare professionals such as hospitals or other agencies.

All children and young people referred to the Children’s Community Nursing service must have a designated consultant paediatrician managing their on-going care.

Moving from Children’s Community Nursing (CCN) to an Integrated Community Team (ICT)

There are some changes when you move from children’s to adult services. This happens when you turn 18 years old.

We organise ICTs by area. These are Cheltenham, Cotswolds, Gloucester, Stroud and Forest, Tewkesbury, Newent and Staunton (Forest TNS).

The way we arrange care changes when you move to adult services

Annual health checks

Are you are on the Learning Disability register with your GP? If so, ask your GP for an annual health check.

 

Enteral feeding

You can get feeding supplies and enteral feeding tubes from the Adult Home Enteral Feeding Team. They can give you NGTs and Gastrostomy buttons.

Gastrostomy Buttons and PEGs

Do you or your carer change your button or manage your PEG?

For help with concerns or equipment please contact the Adult Home Enteral Feeding team.

 

Do you need support with button changes every three months?

If so, the children’s nurses will talk to the adult nurses about your care. Please then contact the ICT to arrange an appointment.

 

Do you need help with PEG care?

The children’s nurses will train you or your carer to do this. If you need more help the ICT will visit you each week.

 

Nasogastric tube care / Nasojejunal tube care / Gastro-Jejunostomy tube care

The Home enteral feeding team will help you with these. Please contact them if you have any concerns or need equipment.

 

Ordering Syringes

Do you manage your button or gastrostomy care? If so, please contact your GP surgery to order syringes.

The CCN will tell your GP the syringes you need.

Is our ICT team providing gastronomy care? If so, they will provide syringes. Please contact them on the numbers above to order your supplies.

 

Venepuncture / Bloods

Your ICT can do blood tests if you cannot go to your GP. Please contact your ICT team to ask for a test.

 

Pressure Ulcer Care and management / Wound Reviews

Your ICT can help with pressure care and wound care. If you need a review at home, please contact your ICT.

 

Portacath and Central Line Access

Our CCN team will arrange some joint visits with your ICT nursing team. They will train them to access your port or central line. They will do this when you are 17 1/2 years old. After this, please contact the ICT for your monthly port access appointments. They will also help with routine bloods as needed.

 

Subcutaneous and Intramuscular Injections

We will train you to give yourself injections.  We can also train a family member. If no-one is able to do this you should contact your GP for a regular appointment. If this is not possible, the ICT will help you.
