Home First

Home First is a service to support you to return home once your treatment has been completed and you no longer need to stay in hospital. We will work with you to continue your recovery in your own home so you regain your ability to live as independently as possible.

What does it involve?

We will help you settle back at home, establishing your routines and support networks, to live well and plan your next steps. We will work with you to establish what you can do by yourself, and what you would like to achieve, with, or without support.

Areas of support that we might help you with include:
• Washing
• Dressing
• Cooking meals
• Accessing community services
• Arranging shopping or medication
• Improving your mobility to safely use steps or stairs
• Walking or using transport

You may have a particular hobby or interest, that is important to you. We could help you to get back to do the things you enjoy. Depending on your individual need you may receive support from therapists, community nurses, other community or voluntary services. We will work with you and anyone who supports you, your family, carers or informal support, to help you live well at home. Your Home First plan will be regularly reviewed with you and updated as you progress.

Service times

The service operates from early in the morning until evening, 7 days a week including on bank holidays.

Will I have to pay?

Home First lasts for 1 to 10 days and is funded by the NHS.

Moving On

We will work with you to discuss your progress and help plan your next steps.

This might include:

• Being discharged from Home First to live independently
• Signposting to community support
• Referral for ongoing community therapy
• Further rehabilitation
• Referral to Adult Social Care for a Care Needs Assessment
