Homeless Healthcare Team
and Special Allocation Scheme
There is a recognised population of homeless people and families across the county who find it difficult to access primary health care because of their circumstances and lifestyle.
Our Homeless Healthcare Team offers permanent registration for those patients with potentially complex and high levels of need, whilst providing a safe environment for homeless people to access primary health care.
Our aim is to promote self-care, whilst improving access to mainstream mental and physical health and social care services.
