Rapid Response

We’re an urgent care service, working 24 hours a day, 7 days a week specialising in management of acute illnesses in adults within the community, preventing unnecessary admission to acute hospitals. Referrals are triaged and prioritised to ensure we can provide prompt, safe and effective care to our patients.

We have access to:

• non-medical prescribers
• near patient testing, diagnostic and blood analysis
• the provision of home oxygen therapy and nebulisation treatment.

We work with:

• Frailty Assessment Service
• South Western Ambulance Service Trust
• palliative care teams
• Heart Failure, Diabetes and Community Respiratory and District Nursing teams
• out of hours and patient’s own GP.

Criteria for referral

• Referrals can only be made by registered practitioners, for patients presenting with acute illness.
• Patients must be registered with a Gloucestershire GP.
