Primary Care Liaison Service Feedback Form
The purpose of the Primary Care Liaison function is to provide a single point of contact to identify and resolve issues at the interface between the GHC services and primary care teams, allowing a two-way dialogue to raise concerns and to identify and address themes.
Examples of categories for feedback are:
- Fit notes and inappropriate tasks: Requests for Fit Notes, investigation requests and action on non-incidental tests blood tests that should have been completed by secondary care.
- Onward referrals: Requests for primary care to enact onward referral for conditions related to the original referring complaint and within specialists’ scope of professional practice, instead of being handled directly by GHC.
- Clinic Letters & Discharge Summaries: To clarify unclear responsibility for follow-up actions, poor formatting or ambiguous communication.
- Referral Rejection Queries and discharge decisions: Questions where the GP believes the person’s needs were appropriate for the service should initially be directed to the relevant clinical team.
- Referral Rejection Queries: Questions regarding the rejection of a referral where the GP believes the referral was appropriate for the service.
This form is not for:
- Clinical queries or patient management decisions
- Requests to expedite referrals or planned follow-up due to change in clinical status – this should be directed to the responsible clinical team.
