The below information is required, so we can understand your goals, your reason(s) for choosing a particular course (or courses), as well as your support needs to ensure you are on the best course for you. Please be aware that our criteria varies, depending on which course(s) you select – please see the notes in the course boxes below. You must be aged 18 years or over and be registered with a GP in Gloucestershire to enrol on one or more of the below courses. If you are unsure whether you meet the criteria, please contact us for more information.

What to expect

Once you have completed this form and sent it to us, we will let you know that we have received it. You will find the course closing date in our prospectus – please make sure your form is back to us by this date. Forms received after this date will not be considered for a place at this time. Adhering to the closing date allows us to process interested applicants, get in touch if we require further information or need to make additional plans to support you attending our course.
 
After the closing date the team will meet to look at all the application forms. As a health education service, we also look at clinical notes and risk assessments so we can ensure we are able to fully support you on our courses. We may also get in touch with you and health professionals you work with to further develop personal support plans.
 
Any information you provide is treated confidentially, safely stored and disposed of in accordance with data protection. We will then contact you to confirm your place and send you any pre-course information.
 
If you are not allocated a place we will let you know and discuss ‘where next’. You may be added to a waiting list if the course is full, or if you do not meet our criteria, we can signpost you to other services that might be able to help
