Team of the Week: Integrated Community Teams (ICTs)

Working as a united team to serve local areas, our Integrated Community Teams (ICTs) are formed of Community Nurses, Occupational Therapists, Physiotherapists, Social Workers, and Reablement Workers. These teams also work in tandem with GPs to provide care for people in their place of residence.
